Cramps that have gotten worse over years instead of better. Pain during sex (deep dyspareunia). Pain with bowel movements during your period. Pain with urination during your period. Very heavy or long periods. Infertility. Chronic pelvic pain outside your period. Fatigue that does not match how much you sleep.
What to ask for at the appointment
A pelvic exam by someone who knows what to look for. A transvaginal ultrasound (looks for cysts, adenomyosis). MRI if ultrasound is unclear. Referral to a gynecologist or endometriosis specialist if your GP brushes you off.
Treatment ladder
Continuous hormonal contraception (skipping placebo) for mild-moderate disease. Progestin-only options if estrogen is contraindicated. GnRH agonists (Lupron) or antagonists (Orilissa) for severe pain not controlled by other meds. Laparoscopic excision surgery (NOT ablation) by an endometriosis specialist for definitive treatment of severe or fertility-affecting disease.
What does not work
Pregnancy as a "cure" (it is not). Ablation surgery (high recurrence vs excision). Going gluten-free as a stand-alone strategy. Suffering quietly.

Can endometriosis be diagnosed by ultrasound? +
Sometimes (cysts, adenomyosis). Definitive diagnosis is laparoscopy. Many specialists can make a clinical diagnosis from history + exam + imaging now.
Does endometriosis cause infertility? +
It can, depending on stage and location. Many women with endo conceive without intervention, others need excision surgery or IVF.
Is endometriosis hereditary? +
Partly. First-degree relative with endo raises your risk 5 to 7 times. Worth telling your daughters and sisters.
